About Baycare Health Partners, Inc.
Baycare® is a Provider Hospital Organization (PHO) serving the four Baystate hospitals and close to 140 medical practices throughout the Pioneer Valley of Western Massachusetts including Hampden, Hampshire, and Franklin counties. Our membership includes approximately 1,300 physicians, along with approximately 750 advanced practice clinicians.
We are dedicated to supporting our members through health care education, quality improvement support and resources, and managed care contracting services.
Our History
For more than four decades, Baycare Health Partners has helped providers and hospitals in Western Massachusetts improve outcomes for their practices. Baycare™ began as an alliance to achieve favorable rates for providers, and now is proactively involved in helping practices perform more efficiently and effectively, leading to better clinical outcomes and cost savings.
Today, our organization includes three distinct entities working together for your patient populations.

- Pioneer Valley Accountable Care, LLC – for Medicare patient populations
- Baystate Health Care Alliance, LLC – for Medicaid patient populations
Baycare®
Baycare® is an alliance of the medical staff and Baystate Health hospitals designed to improve the quality, safety, efficiency, and sustainability of health care in our community.
The economies of scale afforded by this collaboration enable member practices to manage patient populations cost-effectively while continuing to adhere to best practices and deliver the highest levels of care.
Approximately 70% of our staff are Nurse Care Managers and Certified Medical Assistants (Care Coordinators) embedded within healthcare practices. Our clinical integration programs foster and support collaboration among providers ensuring that best practices are freely shared, practice processes are evaluated and modified as needed, decision-making is evidence based, and redundancy is minimized.
Pioneer Valley Accountable Care (PVAC)
Formed in 2012, Pioneer Valley Accountable Care (PVAC) is a single member limited liability company in Springfield, Massachusetts, fully compliant for Medicare providers.
PVAC operates as an accountable care organization. PVAC provides care for approximately 40,000 Medicare beneficiaries, primarily seniors and some disabled beneficiaries younger than 65. Approximately 20 percent of these beneficiaries are dually eligible for Medicare and Medicaid.
Pioneer Valley Accountable Care participated in the Medicare Shared Savings Program (MSSP) between 2013 and 2015 and the Next Generation ACO Model from 2016-2021. We are excited to once again participate in the Medicare Shared Savings Program beginning in 2022.

Baystate Health Care Alliance (BHCA)
Formed in 2017, Baystate Health Care Alliance (BHCA) provides fully compliant support for Medicaid providers.
BHCA represents five community health centers and has partnered with Health New England, a managed care organization owned by Baystate Health, to form the BeHealthy Partnership, which participates in the MassHealth Accountable Care Organization (ACO) program. These health centers encompass seven sites of care in Springfield and have come together to achieve sustainable delivery system transformation to improve the health and lives of our members and their community.
